We’re making some changes at YHP

At Your Health Partnership (YHP) we are always working hard to make our service the best that it can be, balancing the needs of our patients, the needs of our team, and our contractual responsibilities.

By listening to you, our patients, we know that we have some work to do to make things better for the people that use our service.

Review of the Anima system – 7th May


From Wednesday 10th July 2024, we are launching our new clinical model
which will change the way that patients get help at YHP; the new model focuses on the following key areas:

  • Care Navigation – helping patients to access the right service for their needs, this includes services that sit outside of YHP e.g. Pharmacy First, Sexual Health services, Minor Eye Conditions Service.

  • Continuity of care – helping patients to speak to the same clinician if they are experiencing an ongoing problem so they only need to tell their story once.

  • Different ways of accessing us – giving patients different ways to get in touch with us; online via our website or the NHS app; in person at reception; on the phone.

  • Moving away from everything on the same-day and giving options to book in advance or simply ask a question.

  • Vulnerable groups – giving dedicated access to patients whose circumstances make them more vulnerable.

We plan to launch our new model in stages and will communicate with our patients before each stage to tell you exactly what is changing.

What is changing from Wednesday 10th July:

We will stop using Anima and we will start using SystmConnect:

  • On Tuesday 9th July, all existing requests in Anima will be completed and the system will then show as closed.

  • On Wednesday 10th July, patients will be able to start using SystmConnect to send their requests to the practice

Patients will be able to book a small number of face-to-face appointments directly with a GP, up to six weeks in advance, in person at reception, via the telephone or using the NHS app:

  • There will only be a small number of appointments available across each site and with each available GP

We encourage you to download and use the NHS app:

  • Book appointments

  • View information from your clinical record including test results

  • Order your repeat medication

  • Send a message to the practice
