Frailty Questionnaire

As our population ages an increasing number of people are facing challenges to stay healthy and independent. We want to see if our older patients would benefit from new types of services from the practice and the wider community in Rowley Regis.

Thank you for completing the questionnaire, this will tell us if you are one of these people.

There are seven ‘yes or no’ questions. Your answers will be recorded in your electronic medical record and will remain confidential in the same way as any other health information.

We want to identify people who are living with ‘frailty’ or who are at risk of becoming frail. Frailty is a medical word relating to how the aging process and health problems affect people and their physical resilience. If someone is living with frailty, it doesn’t mean they lack capacity or are incapable of living a full and independent life. When used properly, it actually describes someone's overall resilience and how this relates to their chance to recover quickly following health problems. We are focusing on patients aged 65 years and over who live in Rowley Regis. We plan to try out some new types of medical review appointments and invite our patients to some of the different opportunities happening in the community in Rowley Regis to improve health and wellbeing. We will be contacting individuals in the coming weeks whose questionnaire responses suggest may benefit from this approach.
